My ACTION Tracker will keep track of patients’ weights, functional status, heart rates, respiratory rates, rhythm, sleep, and more. This data will be used to better understand the status of our patients with heart failure and will help ACTION to improve care and outcomes.
My ACTION Tracker integrates with and shares data with the Apple Health app and the HealthKit API.
